Our agent is Mr. Mike Bell. The number to call is 675-9494 to ask for a claim form. Please call as soon as possible and report the accident to them so you can be on record. Do not have the parents wait until bills are paid, or until the season is over. The boys insurance goes into effect on February 1^st of each year. The girls insurance goes into effect on July 1^st of each year.
